I get requests for these cards on a frequent basis, so I thought it best to post directly. Definitely not perfect but at least they are logical. Printed on A4 paper, cut (horizontal) fold (vertical) and plastify ... and you should have a set of cards ready to go. My linked in page has comments on how to use the codes, rock and drill hole log forms and perhaps a few more odd's and sods which I have forgotten about.
